wellnesspathgo facilitates access to the DOSEVANA platform and supports the initial patient intake for medically supervised wellness programs, including weight management and men's and women's health solutions. Completed intake information is made available through the DOSEVANA platform for review by independent licensed healthcare providers and, when appropriate, for fulfillment through licensed pharmacies.

2. Eligibility

You must be at least 18 years old to use our services. Minors may only access services under the direct supervision of a legal guardian and healthcare provider.

3. Medical Disclaimer

wellnesspathgo is not a medical provider and does not offer direct medical advice, diagnosis, or treatment. wellnesspathgo facilitates access to the DOSEVANA platform and supports the initial patient intake. Medical services, including review of intake information, clinical evaluation, diagnosis, treatment decisions, and prescriptions, are provided solely by independent licensed healthcare providers using the DOSEVANA platform.

4. User Responsibilities

You agree to provide accurate personal and medical information during the wellnesspathgo-supported intake process for proper evaluation and treatment. Intake and care-workflow information is processed through the DOSEVANA platform and made available, as needed, to the independent licensed healthcare providers, pharmacies, and service providers involved in delivering the requested services. Misrepresentation of medical history may result in termination of service access.

5. Orders, Payments & Refunds

Payments are processed via Stripe. Products are non-refundable once shipped unless otherwise stated due to damage or prescription error.

6. Intellectual Property

All website content, graphics, and branding are owned by wellnesspathgo and/or its licensors. Unauthorized use is strictly prohibited.

7. Limitation of Liability

We are not responsible for adverse health outcomes or misuse of prescribed medications. Always consult your physician for personalized guidance.

8. Governing Law

These terms are governed by the laws of the United States and the state in which wellnesspathgo is registered.

9. SMS Messaging Terms & Conditions

As part of services accessed through the DOSEVANA platform, patients may receive essential SMS notifications regarding their orders, shipping updates, prescription follow-ups, and telehealth visit reminders. These messages are strictly transactional and are sent only to users who have provided explicit consent.

How Users Opt In:

Patients opt in to SMS notifications during the wellnesspathgo × DOSEVANA intake and order workflow inside our secure application. When entering their mobile phone number, users are shown a clear SMS consent disclosure and must acknowledge it before continuing.

Consent Text Displayed at Opt-In:

"By providing your phone number, you consent to receive SMS notifications from wellnesspathgo. These messages may include order updates, shipping status, prescription updates, and telehealth reminders. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for support."

Opt-Out:
